This website is maintained by United States Fire Insurance Company on behalf of itself and its affiliated companies within Crum & Forster (collectively, “C&F” or “Crum & Forster”) and these Privacy Principles guide Crum & Forster’s conduct in the collection, use, release and security of personal and confidential information it obtains as part of its business of providing, producing and servicing commercial and personal lines insurance products, including underwriting, policy administration, insurance claims adjusting, production, appraisal and loss control services. These principles define Crum & Forster’s commitment to the privacy and integrity of the information it accumulates, manages and stores.
Who collects and has access to non-public personal information?
Personal information may be collected by and/or shared with employees of DMC Insurance, Inc. (“DMC”) or C&F or by any of DMC’s or C&F’s authorized representatives, attorneys, or others who provide services to DMC or C&F in connection with providing, producing and servicing its commercial and personal lines insurance products, such as claims administrators, independent appraisers, managed care providers, systems vendors, or similar service providers. DMC and C&F require service providers to honor the privacy principles in the handling of non-public personal information obtained through its business relationship with DMC and C&F. Additionally, DMC or C&F may disclose information to third parties as allowed or required by law. For example, information may be released to comply with a statutory reporting requirement or in response to a subpoena or other order or inquiry of a court, regulator or governmental agency or to fulfill DMC’s or C&F’s obligations to its insurers or reinsurers.
Why do DMC or Crum & Forster need personal information and what do they do with it?
DMC and C&F limit the collection, disclosure, and use of customer information to only what is needed to properly produce, underwrite and service its insurance products, and/or to fulfill legal or regulatory requirements. DMC and C&F collect personal information solely for conducting its business of producing, underwriting, servicing and administering its insurance products including, but not limited to:
- Producing business through its producers
- Underwriting and renewal of its commercial and personal lines insurance products
- Claims handling and adjusting, including investigation and payment of claims
- Claims administration and reporting
- Fraud detection and prevention
- Loss Control
- Complying with the law and statutory reporting requirements
- Business activities that DMC and C&F may legally undertake
DMC and C&F do not sell information to any third parties.
What types of information are collected?
The type of information that DMC collects varies according to the insurance product involved, and may include information DMC receives from you on applications and other forms; information DMC receives from your employer; information DMC receives from other sources such as motor vehicle reports or other insurance support organizations or medical providers.
Safeguarding Your Privacy
Access to non-public personal information is limited to those employees, and authorized representatives, attorneys and service providers who specifically need such information to conduct their business responsibilities. If you conclude your relationship with DMC, DMC will continue to safeguard your privacy in accordance with the standards described in this notice. DMC maintains physical, electronic and procedural safeguards to protect non-public personal information.
